Cancellation of Health Policy from my ICICI Bank Credit Card and Refund of Money

ICICI LOMABARD issued a health policy to Mr. Mohan Ganguli Sharma in Dec'08 and EMIs of this policy is being deducted from my Credit Card Account. I have no link with Mr. Mohan Ganuli Sharma. His address and Phone Nos. are also not matching with undersignedTill date I have also not received any policy from ICICI LOMBARD. I have made several phone calls and written complaints to ICICI BANK & ICICI LOMBARD, but there is no action till date. I have registred the following complaint nos from ICICI LOMBARD:

Policy Document and Cashless card not received

Misselling of Life Insurance Policy

My complaint is against ICICI Prudential Life Insurance POLICY NO-10606869 which i had applied on 10th Dec08 and submitted 6000 for first two installments, but when i received the documents on 25th dec08 i found that all the personal details are wrong, also the nominee details are not correct at that moment GAURAV SHARMA AGENCY MANAGER FROM KAUSHAMBI BRANCH convinced me that they help me to cancel this ploicy and also tells me that this policy is not beneficial for me, on 23rd jan09 we register a complaint against this policy in Kaushambi Branch, but until upto now no results come and when i enquire about this from customer care, they answer me that there is no any complaint register against this policy also the policy is in lapsed state and if i try to cancel this ploicy then i am not getting any money from this policy.

I ALSO SUBMIT A LOAN-EMI CANCELATION LETTER TO SEC.-16 FBD. HDFC OFFICE BUT STILL NO ACTION.
I CALL IN CREDIT CARD DIVISION THEY TOLD ME TO FAX THE POLICY CANCELLATION LETTER TO CREDIT CARD DIVISION BUT HDFC STANDARD LIFE SEC.-16 OFFICE REFUSED TO GIVE ME THE LETTER.

Claim Not Received

Dear Sir,

My ICICI Lombard Health Insurance Policy Number is 4063/FHR/0350/1602/00/0000

I have made 3 claims related to my Heart Problem followed by Angiography and Bypass Surgery to TTK Health Services who the TPA of ICICI Lombard Health Insurance.

Please note that my claim numbers are as follows

Please note that I received a letter from TTK saying that there was some shortfall of documents in one of the claims. The documents requested in the letter were not at all originally required, but still I provided all the necessary documents hoping for some positive action from them. They received my shortfall documents on 29th Jan 2009 and told me that it takes 7 working days to process those documents. I waited till 6th Feb and then starting from 7th Feb I have been calling them every alternate day and they tell me that the documents are under process. They always say that I should get some answer in 2 days. This way I have lost another 1 month and still I am not getting any positive reply.

I hope to solve this problem by reporting this to you.
